From April 1st, 2008,
incoming exchange
students could
fulfill the
requirement of NTU
Health Exams (Form
I) according to the
following methods:
1. they may get
the exams done in
their home country
and mail the Health
Exam Form to our
office with other
required documents
by the application
deadline.
2. If students
couldn't receive
their exam reports
before they send out
the application
documents, they may
a. fax the copy
of the Exam Form and
then mail the
original one to our
office before their
arrivals to NTU.
b. fax the copy
of the Exam Form and
the signed
Health Exam
Declaration to
our office first,
and then bring the
original one to
Taiwan, hand in the
form to us in person
on the Registration
Day.
3. If students
couldn't complete
the health exams in
their home country,
they should first
send us the signed
Health Exam
Declaration, and
then do the exams at
the National Taiwan
University Hospital
once they arrive at
Taiwan. Please see

for the
Instructions of
Health Exams at NTU
Hospital.
Please note that
exchange students'
failure to provide
the NTU Health Exam
Form to Office of
International
Affairs by the
Registration Day
will result in a
failure to complete
the registration
procedure. They will
not receive their
NTU student ID cards
until all the
required documents
are provided. |
